Code Lyoko is a French animated TV series created by Thomas Romain and Tania Palumbo and produced by the MoonScoop Group. Although it has been many years since Code Lyoko was on TV, there is still an active fanbase for this show all over the world and many regions have had official DVD releases, along with official youtube releases of various dubs. Unfortunately, Japanese Code Lyoko fans were never able to see the entire show. Only the first two seasons and the X.A.N.A Awakens Special of the Japanese dub were aired on DisneyXD in Japan. There was never a DVD release, so the few episodes we have found so far were recorded by Japanese fans.
We know that the show wasn't aired to completion in Japan because of DisneyXD's TV schedule logs, wikipedia, lost media archive/wiki, and various Japanese cartoon bloggers. Also, Bashi; a Japanese member of our translation group, has told us that the show was cancelled and based on his own research the show was never fully dubbed into Japanese. He's the one that uploaded all the episodes of the dub that we have now to his youtube channel: www.youtube.com/channel/UCnqzeXhQDV0bRquhQ3wznrg We hope to find all of the episodes of the Japanese dub, Disney Japan has been contacted about releasing the Japanese dub on Disney+ Japan but they simply stated the won't/couldn't do that.
Our translation group's plan is to translate all of the Japanese episodes that we have found so far and also to add Japanese subtitled to the remaining English episodes that were never dubbed into Japanese. With Kad-san as lead translator (he's brilliant), Bashi-san and I will be working on adding Japanese subtitles to the English dubbed episodes so that Japanese fans can finally watch the entirety of Code Lyoko. dont have much to add but this looks interesting. also, i looked on kiyubaru's channel, and they last uploaded 3 weeks ago. some of their stuff seems to be from the 80's, so maybe there's a chance they'll keep uploading more stuff.
Yeah, hopefully it'll be just a matter of time until Hiroshi Harada gives the okay to kiyubaru to post Limitless Paradise onto youtube. I can attempt to send them a tweet in Japanese asking them about it.

I feel like we’re incredibly close to finding Limited Paradise as well as other obscure works by this group, after looking through some of the videos I wouldn’t be surprised if this obscure group of animators also created something similar to Saki Sanobashi.

So far I haven’t been able to find any news coverage like this for Yasuko Endo. If we could find a television memorial programs like this perhaps there’s a chance that they played a clip of “In the Distance” or perhaps interviewed her managers or representatives from Pony Canon regarding when they decided to not release the single.
It’s unfortunate that there is no English wikipedia page for Yasuko Endo, but she is mentioned as a host of several TBS Radio programs. en.wikipedia.org/wiki/TBS_Radio The program she hosted solo is 『歌のない歌謡曲』I’m unsure if the show was canceled after her passing, or if it was continued with a new host. I'd like to do what I can to help you with this search. I'm not a professional level translator (yet) but I have a decent level of Japanese reading and listening comprehension. I'm a fan of city pop and also many other genres of Japanese music too. After listening to Tetsuo Sakurai's cover of "In the Distance" I felt even more determined to help with this search. It's a beautiful song, with a lovely arrangement of jazz instruments and I'd love to hear Yasuko Endo's voice singing "In the Distance."

(The title is "Endo Yasuko/ Pack in Music" which I'm guessing was a radio show. I could be wrong but I think this is is a clip of announcements for upcoming live shows.) EDIT: Just realized I misread the Kanji, this is a radio show of 遠藤泰子 ja.wikipedia.org/wiki/%E9%81%A0%E8%97%A4%E6%B3%B0%E5%AD%90, her name is also read Endo Yasuko, but she is not 遠藤康子 <-the idol Yasuko Endo. The second to last kanji character is different, I should have noticed that sooner.^^; My mistake.)
Endo is mentioned along with many other Japanese young people that committed suicide. Some were idols and there seemed to be a sad domino effect called "Yukiko Syndrome", named after the idol Yukiko Okada, which is when devastated fans of an idol that committed suicide would also attempt it, sometimes going to the same rooftop or location that their idol was. It's incredibly sad to me and as I have experienced the aftermath of when a loved one commits suicide my heart goes out to Endo's family and all of the families that have experienced such a loss.
I sincerely hope that we can find this single in remembrance of Endo.

Here's a list/video showing the covers of all of the RPGs released on Playstation 2. That description sounds like a cover I've seen somewhere before. Assuming that the artwork on the disk anyway, the closest cover that looks like what you described is Suikoden IV but I'm not sure the character is female or not, but they clearly wear a headband and it looks like there could be bandages around the legs. Awesome information Yockey! I'm going to read through that pdf and see if I can translate anything else you haven't covered. I hope that we can find a copy of that cd-rom, I would be surprised if an arcade owner in Japan would destroy the disk and/or the machine. I bet they would put it in storage or something like that.
I’m certainly interested in this and have posted this so that the members of our discord will know about it. Pepper Ann is a way underrated series, and I still have hope that we can find all of the lost episodes and perhaps even one day we can at least get P.A. fully released digitally by Disney.
