[S] (Possibly NSFW) Unknown VHS Warning Screen

I'm trying to recall this warning screen I once saw, but I'm having some trouble tracking it down. I remember seeing it when I was watching reviews of some niche horror films online many years ago (around 2013-2014), though I can't place which film this could've come from (or the review thereof). The screen itself was visually similar to the PS2 RSOD, though it was just a static image; a dark red, cloudy void with some white text on it. The version of it I saw sticks out in my mind as having been ripped from a high generation VHS. I remember this screen having a pretty disturbing track accompanying it, not too dissimilar to the music used in those Goldwater law firm commercials: youtu.be/MZoxfV7Ty6c
I seem to recall the reviewer pointing out how unnerving this screen was, but I could be misremembering.

I tried to transcribe part of the track that I remembered on piano. It was very similar instrumentation-wise to the Goldwater track:
