Fire and Flame was meant to be the second half of season 3 of the 4kids dub. It was set to be released by Funimation in Septermeber 2010. After the final episodes aired on tv, Rainbow pretty much yanked the license from 4kids, possibly due to series creator Iginio Straffi not liking the dub. In 2012 the series would be revived and a new dub commissioned by Nickelodeon.
The first half of season 3, a set called The Princess Ball was released in January 2009. However it had a very limited printing due to the aforementioned license revocation, and is very expensive second hand. Seasons 1 and 2 are relatively easy to find.
There are listings for Amazon and Best Buy created before the cancelation. I'm inclined to believe that small quantity or at least prototype set my have been produced.
I heard of something similar happening with a box-set of the Puppet Master series, apparently it got cancelled so last minute that some copies had already been pressed and sent out(forgot why it was cancelled, rights issues I think)making them extremely rare and sought after and they go for hundreds of dollars on Ebay.

Yeah it sounds like it's a similar case to Volume 8 of ADV's Knights of the Zodiac DVD "Brink of the Abyss" which was announced and had an Amazon page but got cancelled at the last minute and never got in stock.

funnily enough, the same type of shit happened in my country, unlike the USA, we simply had a dub of the Italian version, the only thing that was different, was the dubbed voices, the music etc was all the same as the Italian version.
when season 3 was aired in my country, dvd's were released a little later, all dvd's being multiple volumes with 3 episodes each, adverts were made to promote the last 2 dvd's of season 3, volumes 7 & 8, with cover art and even saying "available now"

no, here in Europe they used the Italian version, and simply dubbed over the voices, leaving everything else intact, so Europe never got the 4kids dub.

Not to mention that the current owners have tried to distance themselves from that dub as much as possible.
