So this is a search that has been going on on TOMT for a while, I've been involved and I actually contacted the person who made the post.
There are further details, and literally HUNDREDS of attempts at identifying it (including several who saw it) but the title seems to escape everyone. I have to say, I am virtually certain I saw this too...for some reason I remember a kind of mystery/horror theme akin to Death Note. In fact for a moment I thought it was Death Note, but then I went back and watched it and...nope.
I directed the person to the LMW, when we briefly discussed this, but I also want to get the opinion of the folks around these parts. They went through some possibilities so far, the closest anyone has gotten is a Tumblr post. Having literally NO IDEA how to search Tumblr for anything, or even how to navigate the post in question.
I know I must sound like a broken record, but after some of the big finds people have made just from some Whang! videos in the last few years, I honestly think the chance of "false memory" is almost a Mandela Effect in and of itself. If Super Giles is real then an anime with a weird intro song is basically a given.
So, I'm trying to keep my ear to the ground, and I hope maybe we can some other hats thrown in the ring.
Someone in those Reddit comments suggested checking the TV Tropes category of "Evolving Credits", and that trope does have a sub-page that is specifically for anime and manga. That sub-page definitely isn't comprehensive, because I know of an anime that uses evolving credits but isn't listed there. So those are my two cents.

Mahoraba, but it's not an anime where a lot of people die.
Sort of wish they gave more info about genre of the show, it would probably be easier if we knew if the show as a schoollife, an action, a horror, a fantasy, etc.

I'll ask to see if they can actually give a description beyond that, given the concept of the characters slowly disappearing until only one or two remain I assume horror of some kind but I'm not sure.
