Download mame64_0204+segac2.rar from here, which has the custom version of MAME that I mentioned earlier.
Download the actual game here, you don't need to extract the ZIP file. Just drag it onto the MAME EXE file and it should automatically start playing (though you'll probably want to configure controls first).

This screenshot is from the ending of Activision's infamous 1988 NES "Ghostbusters" video game. And yes, all the typos and simple grammar mistakes are actually in the finished game.
In all seriousness, this is a cool find and I honestly never knew that a puzzle arcade game can be so sought-after by fans of the Sonic the Hedgehog franchise. I bet that this game's finding will have a serious impact on Sega fans in general, too.
