I mixed up a lot with fantasy games-Monster Hunter and Dragon Age, and heck even Gauntlet got mixed in there for some reason. Resistance and Infamous and a few other "darker themed" games also gave me issues.
Actresses: Angela Lansbury, Maggie Smith (RIP), and a few other "Broadway style/drama" actresses used to get blurred together in my head. Maybe playing similar roles?
I for some reason thought that Elden Ring had something to do with the Elder Scrolls series, like it was another sequel like Morrowind? Probably the title threw me off. Nope, Elden Ring is Dark Souls, and Elder Scrolls is...basically Skyrim now. I think.
I watched Princess Gwenevere and the Jewel Riders as a very young child and for many years, I thought it was Sailor Moon. To be fair, they are both magical girl shows.
There was also a show I watched at a kid in the 90s which I thought was maybe Dragonball Z but watching through the show, the plot I remember never happened. The main character looked a lot like kid Gohan. It could have maybe been Dragon Quest, IDK.

I'd say that you accidentally caught the Sailor Moon pilot, but since that never premiered on TV and was only recently unearthed, it's super interesting you would mix up those 2 shows.
I thought that the DIC show The Littles (which used to be on Kids WB, at least here in the states) was a Borrowers adaptation. It's not. Interestingly enough though, not only is The Littles mentioned as a blurb on the Wiki page for the Borrowers book, but reportedly there IS an animated Borrowers adaptation that is either in production or has wrapped up production.
There were a lot of similarities. It had a blonde lead, a moon crystal, transformations, male love interest, cat sidekick. But a key difference was it took place in a fantasy setting and there were horses.
I remember the first time I watched Sailor Moon, I was so confused. I was like, wait a minute, THIS is Sailor Moon??? Why are they in high school??? Where are the horses???

Well, I remember when I first saw Sandybell as a kid, I thought it was Candy Candy because they were both about a blonde girl with an animal companion and that they had similar stories before I realized her name was Sandybell and that I had managed to catch more episodes of Candy Candy later on the same channel though.
