A post from 2020 on the Unidentified Weird TV blog describes a British PIF (Public Information Film) about escalator safety. The initial recollection comes from the Weird British TV Memories Facebook group:
This is considerably different to the more well-known escalator PIF produced by the COI in the 1970s, which illustrated the same danger using wellington boots:
The escalator PIF with the doll, however, has never surfaced despite a fairly large number of comments online from people who remember it. Here's some examples:

There's a few other mentions out there but they all pretty much recall the same details.
It's possible that it's out there in an advert break/continuity video but the chances of a PIF like this appearing without at least somebody taking notice are fairly slim. It's also unknown whether this was a sister PIF to the one with the wellington boot or part of a later or earlier campaign.
Unfortunately there's not much other information available, but it certainly existed based on how many people remember it.
The PIF featured a little girl on an escalator, holding a rag doll. She dropped the doll and tried to pick it up, but the doll was hideously mangled and scalped underneath the bottom step, and the PIF ended with a shot of a man standing holding the doll’s hair.
It doesn’t seem to have run for as long as the ‘welly-boot’ PIF did, and the escalator might have been one of the old wooden escalators that were banned after the Kings’ Cross fire.
