Movie about a dog who turns into a human.

So I vividly remember a movie where a human gets reincarnated into a dog. The plot was something like A Dog's Journey. Which I saw years later. I remember that the dog was brown and a puppy. I also remember a scene with that dog and a grave, with the grave being of the human. Please help for this will haunt me for a long time.
