I remember when I was younger I was at Children’s Hospital. In one of the lobby areas there was at least one arcade unit that played. I played it at least twice to know that the game would freeze every time you had to talk to Hunter for the first time. This to me meant that I went to this lobby on two occasions, as once the game froze, the player could not do anything about it. I don’t know what time frame this was but it would have to say probably before 2012, maybe earlier then that.
Now, I don’t know if this unit was Spyro branded or generic and that will probably make or break this as “lost media.” I want to say it was Spyro branded, but I really can’t recall. It could probably just be a generic unit.

Do you remember anything about the game setup like controllers or etc.?
I was pretty sure it was the same as the gamecube game. And I don’t really remember, but I assume the controller set up had the controller attached to the unit/arcade, but I really don’t recall.
On a side note, is there any gameplay of the flash version that can be seen online?

The staff would switch around games frequently, though I rarely seen the same games on them.
I would assume that the hospital you went to had the same kind of kiosks, but they did not change the games as frequently. Perhaps the game was freezing at the same spot from the disc's heavy use.

That might be it. So, what we see got so far is, GameCube controllers, and ddiscs freezing. I looked up if gamecubes did freeze, and it is a common problem. So we might be able to conclude this was a GameCube kiosk if we get some more solid evidence.
