Seems interesting to me. I'm curious. Was the skull just a generic skull, or a more cartoony skull? If it's a cartoony skull it might of been an error screen with Futurama itself which just hasn't been documented online. Which can happen. Weird example but the M&M's Kart Racing DS seemingly had no evidence of existing until i saw someone bring it up and tried getting it to work. Since Futurama for PS2 is fairly obscure. It might be just an error screen for the game that wasn't documented.
However, if it was a generic/realistic skull. It might be a PS2 error which, if it is. That is one hell of a discovery about the PS2 d
I figured some one could help find this, or at least tell me if it was even real.
This is such a cool mystery, it's immediately caught my attention and shouldn't be too hard to uncover with a trail to follow right from the start. But I'm questioning this line in your post, do you think it could be a false memory and something that doesn't actually exist?
My first guess would be like the others have said, a screen created by the Futurama game and not something from the PS2 in general.

Oop just now saw this. From my memory I believe it was cartoony but it could've been more realistic and I wouldn't be surprised. I'll try my best to make out what it looked like in the form of an art here shortly
Ok, maybe it was another error screen inside the game, maybe “a memory card is not inserted” or “the inserted memory card is corrupted”? I found a picture of a skull from futurama, did the skull on the error screen look something like this?
I think it should be worth mentioning that it wasn't anything crazy. It was simply a screen with a skull at the top of it and some a few rows of text underneath it on like a black (and red gradient??) background
