Apparently, for the 1998 movie Blade (which I haven't actually seen apart from some random scenes on TV), based on the Marvel Comics character, a scene was filmed where a cop discovers the character Quinn's burning body at a club and alerts its presence to the other police. That cop was played by Stan Lee. This would have been Stan Lee's second cameo appearance in a feature-length Marvel Comics film, and his first in a Marvel film that was released theatrically. As mentioned, though, it was cut from the theatrical version of the movie, and has also never been included as a bonus feature, or leaked in any way. Stan Lee had nothing to do with Blade's creation, so that may be why, but he also didn't create Deadpool, Venom, Guardians of the Galaxy, or even Captain America (though he did end up writing his comics), and he appeared in those movies.
Blade can basically be seen as the first modern Marvel movie, seeing as it was released in theaters and Marvel movies started coming out semi-regularly starting with it (X-Men was released two years later, Spider-Man two years after that, after which Marvel really exploded, basically releasing at least one movie per year), and Stan Lee cameos were a respected tradition for many years, so I'd say this is pretty significant.

I've never heard he was going to have a cameo but he gives some commentary on my DVD of the film, talking about how they removed the comic code insignia from three Spider Man issues because of a drug related subplot and because of the mostly positive reactions, restrictions impose by the comic code organization were lessened the following year, allowing a character like Blade to exist in comics for the first time since the 50's.
