Title says it all, I'll start.
I'm going to cheat and split this into two, the reason I'm doing this is because I collect virtual items aswell as physical so I'm unsure if this will count but here we go.
My two virtual items that are incredibly rare are a glitched fallout76 item known as the "Pristine mr fuzzy suit" and a "Crew Shirt" for the character Nea in dead by daylight a piece of clothing only given to developers. (Yes I earned it legit no I will not be answering questions.)
Now the main thing in my physical collection - I'm a massive furry and collect things with anthro characters this is probably one of the reasons why the game "Night in the woods" is my favourite game of all time.
In my collection is the vinyl soundtrack from night in the woods.
Only 250 were made and it is my pride and joy.
So i pass the question onto you all, what is the rarest item in your collections?
Oh! Um for my gaming collection I have the pre-9/11 release of Spider-Man 2 Enter Electro, which was lost for a really long time but discovered 5 years ago, which is funny cause I was sitting on it for like 8 years at that point.
For a Hot Wheels collection I have this Hall of Fame Deora II replica that's a 1:18 based on a 1:1 replica of the original 1:64 casting of the Deora II that has gone down in results when I've recently tried to find one again, it was a $100 well spent, I even got it signed by Larry Wood at an event. If only Nathan Proch can sign it now...

This is going to have me go through my collection again, isn't it? I'll split this up between my physical content and my digital ones that I have archived to make it easier to ensure they don't become lost since I'm not sure which one to choose. The majority of the digital stuff I have as of now is music and soundtrack oriented stuff that hasn't received an official release. I'm sure I'm forgetting a good amount of stuff, so I'll report back if I remember anything new.
The physical, tangible stuff:
A 13th anniversary Toetag Inc shirt. I'm not sure how many were made, but I got one of the last ones they had in their stock during their 2016 Black Friday sale.
Hand signed copy of August Underground (2001) by Fred Vogel, which is now long out of print.
Hand signed August Underground Mordum (2003) and The Redsin Tower (2006) posters by Fred and Shelby Vogel.
Hand Signed The Final Interview (2018) poster by Fred Vogel, having won it in a drawing for the film's very limited 1983 copy run.
A master copy of AGONi (2015) given to me by the director.
A mini Monster Squad poster signed by Andre Gower, gifted to me by an old buddy who worked with him on a film project.
One or two prop posters used in a short film made for a local film challenge. I'm not sure if the director has any more since he moved a few years ago.
The digital stuff:
Crock Pot Abduction's early demos with their original frontman.
A few of Buck-Tick's super early tracks (including some unreleased stuff).
Psycho Pike (1992) + the closing theme.
The raw, uninterrupted opening to The Hook of Woodland Heights (1990).
An early 80s recording from McMurdo Station.
A slew of Joy Junction related content that I could salvage.

Well, I think the rarest movie item I had at one point was a VHS tape with some weird movie that I never got to watch for whatever reason, or another. I think the word "Indian" was on the title of the movie. Now for the rarest video game item, I'd say Sprint Cars: Road To Knoxville for the PS2. I never heard ANYONE out there ever mention this game, even when it came to obscure video games. It's a racing game, that I know, but I just found a copy of this at McKays, & have nobody to talk to when it came to this very game. I just know that it exists, & I have it. Apparently this is also for the PC, & this also had a sequel, but that's all I know. At least with Combat Cars on Sega Genesis, people do actually talk about it outside of the game's fanbase, but Sprint Cars? Doesn't seem to be the case.
