Lost Building Site PSA by SPLAAT named "Game Over"

I've been looking for a safety PSA from the UK by a company named SPLAAT. It's about a girl named Charlotte, I believe, who plays on a building/construction site which eventually leads to her death - her name gets crossed off the register and it shows her grave.
I think the PSA is named "Game Over" judging by some other individuals on reddit who were looking for it. I'd say it was made in the 1980's or 1990's. I watched it as a child at school in England, where I live. The company SPLAAT has a big headed mascot character, you should be able to find the company with a quick Google search.
I don't have many details regarding the contents of the video as I was quite young and don't remember it in great detail, but I remember a scene from a first person view running through a building site, then they would fall, the video would freeze frame and an animation would play with green slimy text splatting onto the screen, reading out the acronym "SPLAAT" with I believe a child reading it out and what each letter meant in the acronym. There was also Charlotte's name being crossed out of the register with it fading in to her grave with her parents stood around it I think.
I don't know how long it was. I would really appreciate if someone would be able to search for this with me, I've been wanting to see it again for years.
