Huh. Movie seems to have had decent reviews, if this is anything to go by, and has a 7.6 on IMDb, so it's pretty curious that I can't find anything else by Ivo Dvorak, the filmmaker, or that nothing survives of it, especially since it was an adaptation of a classic story.
This channel supposedly uploaded the movie but was taken down for copyright infringement, so there is a slight chance this movie is out there in the web. However there is an even higher chance that is was one of those "click the blue link in the description" channels.
Well this explains a few things, this means that it isnt filed as a financial failure and that it is possible to get this movie released. However i do not know how to get in contact with the movies director/writer.
The guy is 79 now and i doubt we can even contact him (especially since he shares a name with some wine tv show host).
i found what might be his facebook: www.facebook.com/ivo.dvorak.940 however this doesnt show his age or that he was a director, plus the pic is of a young guy. I would message him to find out if i had a facebook, but i honestly have doubts it is him considering the only similarities are that he is czech and like photography (a common trait among directors)
However this linkedin page might be his: Even though it has little info on it it does state that he is czech and a director however i need an account to see more.
also, i did a quick google image search of the poster and found the description for the movie "A Swedish adaptation of the Franz Kafka novella. Gregor Samsa, wakes one morning to find himself transformed into a large, monstrous beetle."
and (in the same google image search of the poster) it was number 39 on the scandinavian horror films list prntscr.com/bibj2n
again, i found some more info. i found a youtube video titled "The Metamorphosis of Mr Samsa" and it's one of those silent films. it follows the description of the movie "he transforms into a large, monstrous beetle" so i'm willing to bet this is an official clip from it.
